Significant changes to bone structure have been observed, and transgender women have statistically poorer bone health even before beginning the transition process, possibly due to a lack of physical exercise or other risk factors such as low vitamin D, eating disorders, and substance abuse. Approximately 14% of transgender women suffer from osteoporosis. Transgender women below the age of 50 show increased fracture risk compared to age-matched cisgender women, equal to the risk to cisgender men of equivalent age. Transgender women above the age of 50 have a similar fracture risk to post-menopausal women — higher than that of age-matched cis men. In both cases, trans women's fracture patterns follow that of cis women, suffering long-term stress fractures concentrated in the hip, spine, and arms, typical of chronic low bone mineral density, rather than the fracture patterns typical of external injury suffered by cis men. Negative-pressure wound therapy (NPWT), also known as a vacuum assisted closure (VAC), is a therapeutic technique using a suction pump, tubing, and a dressing to remove excess wound exudate and to promote healing in acute or chronic wounds and second- and third-degree burns. The use of this technique in wound management started in the 1990s and this technique is often recommended for treatment of a range of wounds including dehisced surgical wounds, closed surgical wounds, open abdominal wounds, open fractures, pressure injuries or pressure ulcers, diabetic foot ulcers, venous insufficiency ulcers, some types of skin grafts, burns, and sternal wounds. It may also be considered after a clean surgery in a person who is obese. NPWT is performed by applying a sub-atmospheric vacuum through a special sealed dressing. The continued vacuum draws out fluid from the wound and increases blood flow to the area. The vacuum may be applied continuously or intermittently, depending on the type of wound being treated and the clinical objectives. Typically, the dressing is changed two to three times per week. The dressings used for the technique include foam dressings, sealed with an occlusive dressing intended to contain the vacuum at the wound site. Where NPWT devices allow delivery of fluids, such as saline or antibiotics to irrigate the wound, intermittent removal of used fluid supports the cleaning and drainage of the wound bed. === Indigenous land claims === Belize backed the United Nations (UN) Declaration on the Rights of Indigenous Peoples in 2007, which established legal land rights to indigenous groups. Other court cases have affirmed these rights like the Supreme Court of Belize's 2013 decision to uphold its ruling in 2010 that acknowledges customary land titles as communal land for indigenous peoples. Another such case is the Caribbean Court of Justice's (CCJ) 2015 order on the Belizean government, which stipulated that the country develop a land registry to classify and exercise traditional governance over Mayan lands. Despite these rulings, Belize has made little progress to support the land rights of indigenous communities; for instance, in the two years after the CCJ's decision, Belize's government failed to launch the Mayan land registry, prompting the group to take action into its own hands. The exact ramifications of these cases need to be examined. As of 2017, Belize still struggles to recognize indigenous populations and their respective rights. Racial segregation does not only impact access to social and economic resources. It also has an impact on public health and disrupts access to health care. The impact that racial segregation has health care spills over to the access of substance use services. This leads to Black Americans having a more difficult time when seeking treatment for opioid use. Structural racism has also led to the consistent misdirection of funds and the over-funding of criminal legal systems within predominantly non-Hispanic Black communities. Instead of funding being used to improve substance abuse treatment and prevention, funds have been used to criminalize drugs and impose harsh penalties on Black community members. The policies put in place years ago have led to stereotyping and fear within Black communities that prevents Black Americans from seeking substance abuse treatment.
