Development began with intranasal formulations investigated for erectile dysfunction, but blood pressure elevation limited that route and prompted a switch to subcutaneous delivery. Clinical testing then shifted toward hypoactive sexual desire disorder in premenopausal women, and a subcutaneous product received United States approval in 2019. Later trials examined other populations with mixed results, and questions about effect size, durability and patient selection remain open in the peer-reviewed literature. Research interest continues in parallel with the broader melanocortin field, where several synthetic analogues are studied together.
PT-141 is the research code for bremelanotide, a cyclic heptapeptide derived from alpha-melanocyte-stimulating hormone. The molecule belongs to the melanocortin receptor agonist family and shows highest affinity for the MC4 receptor subtype, with weaker activity at MC1, MC3 and MC5. Its structure retains the core His-Phe-Arg-Trp sequence that defines melanocortin recognition, while cyclization and terminal modifications improve metabolic stability relative to the parent hormone. Early work classified the compound as a centrally acting agent rather than a peripherally acting vasodilator, which shaped subsequent development priorities.
Published studies differ in design, population and endpoint definition, so results are not always directly comparable across reports. Some trials used patient-reported measures of desire and distress, while others tracked physiological or behavioural outcomes. Questions that remain open include the durability of effects beyond short follow-up periods, the frequency of transient blood pressure elevation observed after administration, and whether a subtype-selective analogue could separate central effects from pigmentation-related activity. These points are usually framed as unresolved rather than settled in review literature.
Routine characterisation of bremelanotide relies on reversed-phase high-performance liquid chromatography with ultraviolet detection near 214 nm, where the peptide backbone absorbs. Mass spectrometry, typically in tandem mode with electrospray ionisation, confirms identity and supports quantification in biological matrices. Additional checks include amino acid analysis, peptide mapping after enzymatic digestion, and confirmation of the lactam bridge, since incomplete cyclisation produces a mass-shifted by-product. Purity values above 95 percent are common in reference-grade material, though reports vary in how strictly related substances are resolved from the parent peak.
The lyophilised solid is relatively stable when kept dry, protected from light and held at reduced temperature, commonly minus 20 degrees Celsius or lower for long-term storage. In solution the peptide is more vulnerable: tryptophan oxidation, hydrolysis of the lactam bridge and aggregation all become relevant over time, and the rate depends on pH, buffer composition and concentration. Repeated freeze-thaw cycles are generally avoided because they promote aggregation. Aqueous working solutions are typically prepared fresh or split into single-use aliquots to limit degradation before analysis.

Bremelanotide acts as an agonist at several melanocortin receptors, with the strongest reported activity at MC4R and measurable activity at MC1R and MC3R. Because MC4R is expressed in hypothalamic and limbic circuits, the proposed mechanism links receptor activation to modulation of central pathways involved in desire rather than to direct effects on peripheral genital tissue. The precise downstream steps remain incompletely characterized, and evidence for the involvement of specific neurotransmitters is suggestive rather than settled. Nausea and blood pressure elevation reported during trials are consistent with melanocortin signaling outside the intended target circuit.
Compared with melanotan II, bremelanotide is a smaller cyclic peptide with a more constrained backbone, which affects receptor selectivity and metabolic stability. Published descriptions give a plasma half-life on the order of a few hours after subcutaneous administration, with elimination through hepatic and renal routes and limited plasma protein binding. Central access is inferred from effects observed in animal models, although direct measurement in humans is limited. Handling and storage requirements follow from the peptide backbone, which is susceptible to hydrolysis and oxidation.
The melanocortin system comprises five G protein-coupled receptors, designated MC1 through MC5, that signal mainly through cyclic AMP accumulation. MC1R and MC2R are associated with pigmentation and adrenal steroid production, while MC3R and MC4R are expressed in the central nervous system and influence energy balance and behavior. MC5R appears in exocrine tissues. Natural agonists include alpha-melanocyte-stimulating hormone and adrenocorticotropic hormone, and endogenous antagonists such as agouti-related protein modulate the same sites. This receptor family provides the framework within which bremelanotide activity is described.
Identity and purity testing for this peptide typically relies on reversed-phase high-performance liquid chromatography with ultraviolet detection, reported as area-percent purity. Mass spectrometry, usually in tandem mode, confirms molecular mass and supports quantification in biological matrices. Sequence confirmation may use peptide mapping after enzymatic digestion, while nuclear magnetic resonance and circular dichroism supply supplementary structural detail. No single technique establishes identity alone, so laboratories compare retention time, mass, and fragment pattern against a verified reference standard.
The lactam ring that closes the peptide backbone improves resistance to exopeptidase attack, but the molecule stays susceptible to hydrolysis and oxidation once dissolved. Degradation accelerates with temperature, extreme pH, and light exposure, and repeated freeze-thaw cycles promote aggregation and material loss. Lyophilized powder held desiccated at or below minus twenty degrees Celsius is the common way to keep reference material. Reconstituted solutions are generally kept cold and used within a short window because their stability is far lower than that of the dry solid.
Activation of MC4R in the hypothalamus is thought to influence dopaminergic signaling, which in turn affects arousal and desire. This mechanism differs from that of phosphodiesterase type 5 inhibitors, which act primarily on vascular smooth muscle in the genital region. Because the pathway is central rather than peripheral, effects are not strictly dependent on local blood flow. The precise downstream cascade linking receptor binding to behavioral outcomes remains an area of ongoing investigation.
Clinical development of bremelanotide proceeded through several reformulation attempts. An early intranasal version was discontinued, and a subcutaneous auto-injector formulation later received approval for hypoactive sexual desire disorder in premenopausal women. Approval decisions have varied by country and over time, and the product has not been universally adopted. Blood pressure elevation is a documented effect, which is why some jurisdictions require monitoring after administration. The clinical evidence base continues to evolve as additional studies are published.
Bremelanotide is a cyclic heptapeptide that acts as an agonist at melanocortin receptors. It binds MC1R, MC3R, MC4R, and MC5R, with MC4R activation considered most relevant to sexual desire pathways in the central nervous system. The molecule is a synthetic analog of alpha-melanocyte-stimulating hormone, a naturally occurring peptide involved in pigmentation and energy regulation. Early research explored its use in tanning before attention shifted toward sexual dysfunction applications. Receptor binding affinity varies across these subtypes.
On a pressure/temperature phase diagram (see figure), there are curves separating solid from vapor, vapor from liquid, and liquid from solid. These meet at a single point called the triple point, where all three phases can coexist. The triple point is at a temperature of 273.16 K (0.01 °C; 32.02 °F) and a pressure of 611.657 pascals (0.00604 atm; 0.0887 psi); it is the lowest pressure at which liquid water can exist. Until 2019, the triple point was used to define the Kelvin temperature scale. The water/vapor phase curve terminates at 647.096 K (373.946 °C; 705.103 °F) and 22.064 megapascals (3,200.1 psi; 217.75 atm). This is known as the critical point. At higher temperatures and pressures the liquid and vapor phases form a continuous phase called a supercritical fluid. It can be gradually compressed or expanded between gas-like and liquid-like densities; its properties (which are quite different from those of ambient water) are sensitive to density. For example, for suitable pressures and temperatures it can mix freely with nonpolar compounds, including most organic compounds. This makes it useful in a variety of applications including high-temperature electrochemistry and as an ecologically benign solvent or catalyst in chemical reactions involving organic compounds. In Earth's mantle, it acts as a solvent during mineral formation, dissolution and deposition.
=== Transplant === In some cases, people can receive transplants of the pancreas or isolated islet cells to restore insulin production and alleviate diabetic symptoms. Transplantation of the whole pancreas is rare, due in part to the few available donor organs, and to the need for lifelong immunosuppressive therapy to prevent transplant rejection. The American Diabetes Association recommends pancreas transplantation only in people who also require a kidney transplant, or who struggle to perform regular insulin therapy and experience repeated severe side effects of poor blood sugar control. Most pancreas transplants are done simultaneously with a kidney transplant, with both organs from the same donor. The transplanted pancreas continues to function for at least five years in around three-quarters of recipients, allowing them to stop taking insulin. Transplantations of islets alone have become increasingly common. Pancreatic islets are isolated from a donor pancreas, then injected into the recipient's portal vein from which they implant onto the recipient's liver. In nearly half of recipients, the islet transplant continues to work well enough that they still do not need exogenous insulin five years after transplantation. If a transplant fails, recipients can receive subsequent injections of islets from additional donors into the portal vein.

=== Peptidoglycan binding and hydrolysis === PGLYRP2 is an enzyme (EC 3.5.1.28), N-acetylmuramoyl-L-alanine amidase, that binds and hydrolyzes bacterial cell wall peptidoglycan. Peptidoglycan is the main component of bacterial cell wall and is a polymer of β(1–4)-linked N-acetylglucosamine (GlcNAc) and N-acetylmuramic acid (MurNAc) with MurNAc-attached short peptides, typically composed of alternating L and D amino acids, that cross-link the adjacent polysaccharide chains. PGLYRP2 hydrolyzes the amide bond between the MurNAc and L-Ala, the first amino acid in the stem peptide. This hydrolysis separates the crosslinking peptides from the polysaccharide chains and solubilizes cross-linked bacterial peptidoglycan into uncross-linked polysaccharide chains. The minimal peptidoglycan fragment hydrolyzed by PGLYRP2 is MurNAc-tripeptide. The peptidoglycan-binding site, which is also the amidase catalytic domain, is located in the C-terminal PGRP domain. This PGRP domain is sufficient for the enzymatic activity of PGLYRP2, although this activity of the isolated C-terminal fragment is diminished compared with the entire PGLYRP2 molecule. Zn2+ and Zn2+-binding amino acids (His411, Tyr447, and Cys530 in human PGLYRP2) are required for the amidase activity. Cys419 in human PGLYRP2, which is broadly conserved in invertebrate and vertebrate PRGPs, forms a disulfide bond with Cys425 (in human PGLYRP2) and is required for the amidase activity, as this disulfide bond is essential for the structural integrity of the PGRP domain.

A common substrate used in this type of system is glass. Depending if the system is open or closed, there would be either one or two layers of glass. The bottom layer of the device contains a patterned array of individually controllable electrodes. When looking at a closed system, there is usually a continuous ground electrode found through the top layer made usually of indium tin oxide (ITO). The dielectric layer is found around the electrodes in the bottom layer of the device and is important for building up charges and electrical field gradients on the device. A hydrophobic layer is applied to the top layer of the system to decrease the surface energy where the droplet will actually be in contact with. The applied voltage activates the electrodes and allows changes in the wettability of droplet on the device's surface. In order to move a droplet, a control voltage is applied to an electrode adjacent to the droplet, and at the same time, the electrode just under the droplet is deactivated. By varying the electric potential along a linear array of electrodes, electrowetting can be used to move droplets along this line of electrodes. Modifications to this foundation can also be fabricated into the basic design structure. One example of this is the addition of electrochemiluminescence detectors within the indium tin oxide layer (the ground electrode in a closed system) which aid in the detection of luminophores in droplets. In general, different materials may also be used to replace basic components of a DMF system such as the use of PDMS instead of glass for the substrate.

=== Cation exchange === Cation exchange sorbents are derivatized with functional groups that interact and retain positively charged cations, such as bases. Strong cation exchange sorbents contain aliphatic sulfonic acid groups that are always negatively charged in aqueous solution, and weak cation exchange sorbents contain aliphatic carboxylic acids, which are charged when the pH is above about 5. Strong cation exchange sorbents are useful because any strongly basic impurities in the sample will bind to the sorbent and usually will not be eluted with the analyte of interest; to recover a strong base a weak cation exchange cartridge should be used. To elute the analyte from either the strong or weak sorbent, the stationary phase is washed with a solvent that neutralizes ionic interaction between the analyte and the stationary phase.

The code designates bremelanotide, a cyclic heptapeptide melanocortin receptor agonist. It served as an internal research identifier before the compound entered formal clinical development. The code and the generic name refer to the same molecule.
MC4R is generally described as the principal target, based on binding affinity and functional assays. Activity at MC1R, MC3R and MC5R is reported as weaker. Receptor selectivity is one reason the compound was pursued for central rather than vascular effects.
Both are synthetic melanocortin analogues built on a similar cyclic peptide scaffold. Bremelanotide is described in the literature as a metabolite-derived analogue of melanotan II rather than the same substance. The two differ in terminal modifications, which affect half-life and receptor profile.
The peptide is polar and charged, and reversed-phase chromatography with acidic mobile phases resolves it well from related substances. Ultraviolet detection near 214 nm gives adequate sensitivity without derivatisation.
